  • Polycystic Ovarian Syndrome (PCOS) is a condition that manifests when you have two of the following: irregular or no periods, polycystic ovaries on ultrasound, or higher levels of androgens in blood or symptoms (acne, excess facial hair, or body hair growth). If PCOS if untreated can cause difficulty conceiving late in life. Patient’s typical symptoms include but are not limited to difficulty losing weight, facial hair growth (hirsutism), abnormal and/or heavy menstrual cycles, and insulin resistance.   • The potential benefits of weight loss (even 5% of body weight) are incredible and include: improved glycemic control, reduction in cardiovascular risk, cancer risk, overall mortality, blood pressure, cholesterol, improvement in sex hormones, and improvement in overall health and wellbeing. Currently, about 31% of Americans are overweight and 42% of Americans are obese! This is the fastest growing disease state in the United States. Studies have pointed to two causes for this: too much food and too little exercise. At Modern Endocrine, we recommend starting your weight loss journey with extensive screening for insulin resistance, thyroid disease, sex hormone deficiencies, and cortisol excess to rule out any causes that would make weight loss more difficult to achieve. We then take a holistic approach to weight loss by typically recommended lifestyle modifications (dietary changes, intermittent fasting, proper sleep, stress/cortisol management, micronutrient supplementation, and exercise) prior to medications. If medications, supplements, or hormones are needed based on labwork, we help patients weight the risks and benefits of medications based on their hormone and micronutrient profile. We are prepared to discuss weight loss and insulin lowering medications with you if applicable.

  • Insulin resistance is a state in which a given concentration of insulin is associated with a higher than normal glucose level. In this state, cells in your muscles, fat, and liver do not respond well to insulin and cannot easily take up glucose from your blood. As a result, your pancreas makes more insulin to help glucose enter your cells. Over time, cells stop responding to all the insulin (insulin resistance). The pancreas keeps making more insulin to try to make cells responds, but eventually the pancreas cannot keep up, and blood sugar levels rise steadily, ultimately leading to type 2 diabetes. At Modern Endocrine, we are dedicated to catching insulin resistance in early stages. We believe in screening with fasting insulin and glucose levels, and treating any fasting insulin level above 5. If you are found to have insulin resistance we will work with you to lower insulin levels, which will ultimately promote weight loss. Thyroid Care

Our clinic provides expert care for a wide range of thyroid disorders, including hypothyroidism, hyperthyroidism, thyroid nodules, and thyroid cancer. We offer diagnostic testing, medication management, and surgical intervention when necessary to support thyroid health.

  • Hypothyroidism (underactive thyroid) is a condition in which your thyroid gland does not produce enough of certain crucial hormones. Symptoms can manifest in a variety of different ways, however, some common symptoms include constipation, fatigue, dry skin, swelling, and difficulty managing weight.

  • Hyperthyroidism (overactive thyroid) occurs when your thyroid gland produces too much of certain crucial hormones. Hyperthyroidism can accelerate your body’s metabolism, causing unintentional weight loss and rapid or irregular heartbeat.

  • Hashimoto’s disease is an autoimmune disorder where the immune system creates antibodies that attack thyroid cells. This damage can lead to thyroid dysfunction and ultimately hypothyroidism if left untreated.

  • Graves’ disease is an autoimmune disorder that results in the over production of thyroid hormones. Although a number of disorders may result in hyperthyroidism, Graves’ disease is a common cause.

  • FNA biopsy is a procedure in which a small needle is used to extract cells from a thyroid nodule in order to diagnose a benign thyroid condition vs thyroid cancer. This very common procedure is performed in the outpatient setting at Modern Endocrine without sedation or the need for a driver after the procedure. Downtime is typically about 3-5 minutes post procedure only.

  • Depending on the type of disease, thyroid conditions are typically treated with medications. In addition to conventional treatments, research shows that certain supplements may help treat thyroid disease. There are a multitude of supplements that are marketed for “thyroid health,” however, it is important to discuss with your provider prior to taking these as certain supplements may do more harm than good when it comes to thyroid health.

    The thyroid needs the following nutrients for optimal functioning:

    • Vitamin D: vital for healthy thyroid function & reduces risk of cancer (including thyroid cancer)

    • Vitamin B12: vital for detoxification and production of energy

    • Turmeric/curcumin: reduces inflammation and can decrease chance of Hashimoto’s patient’s needing thyroid replacement

    • Selenium: reduced levels lead to a decreased conversion of T4 -> T3 and worsening of iodine deficiencies. Replacing a deficiency can lower TPO (Hashimoto’s antibodies)

    • Iodine: needed in deficient patients only to help promote the production of T4 and T3

    • Zinc: needed for optimal activity of thyroid hormone and deficiencies can lead to lower levels for T3

    • Iron: ferritin levels < 70 can suppress thyroid hormones and worsen hypothyroidism

Hormone Replacement

We specialize in bioidentical hormone replacement therapy (BHRT) to restore hormonal balance and alleviate symptoms associated with hormonal imbalances. Our customized treatment plans aim to optimize hormone levels, improve quality of life, and promote overall health.

  • The pellets used for BioTe are high-quality and custom-compounded with bioidentical hormones. Biote’s Method of hormone therapy provides a consistent release of hormones. When hormones are out of balance, hormone replacement therapy may help relieve a wide array of symptoms, both physical and emotional. Within two to four weeks of a pellet insertion, the hormone therapy can start alleviating unwanted symptoms. Bioidentical hormone replacement therapy aims to relieve the source of the problem, not just the symptoms themselves.

  • Hormone therapy for women has been a common practice to cope with aging for decades. Bioidentical hormones can help women through multiple stages of life including peri-menopause, menopause, and post-menopause. HRT in women can be given in several forms (typically creams and pellets) and several different types of hormones can be used based on the patient’s health and desires (testosterone alone, testosterone and estragon, or testosterone, estragon, and progesterone). Female hormone replacement therapy may help women who are suffering from unbalanced hormones which can manifest as: hot flashes, night sweats, sleep issues, depression, anxiety, irritability, fatigue, vaginal dryness, decreased sex drive, difficulty or increased urination, joint pain, brain fog, hair thinning, hair loss, weight gain, or dry skin. Vitamin & Mineral Deficiencies

Nutritional deficiencies can impact endocrine health and overall well-being. We offer thorough evaluation and management of vitamin and mineral deficiencies, providing personalized supplementation and dietary recommendations to address your specific needs and optimize your nutritional status.

  • Vitamin D3 is essential for: energy production, healthy thyroid function, the immune system, lowering risk of falls by improving balance and muscle performance, maintaining muscle and bone, reducing the risk of cancer, and promoting healthy hair and nails. Goal vitamin d levels are at least 60 or at least 80 in patients with a history of a solid organ cancer, but levels should not be higher than 100. About 30% of the population has a vitamin d polymorphism, and they will have to take huge doses of vitamin d to get adequate vitamin d levels. High vitamin d doses WITHOUT K2 can increase micro calcification of vascular beds. Vitamin d is a fat soluble vitamin and should be taken with OrthoMAX or healthy fats (avocado, peanut butter, salmon, eggs) to aid in absorption. Typical Dosing: If vitamin d level is < 40 start Dynamic D-K2 2 tabs daily for 3 months then transition Dynamic D-K2 1 tab daily for maintenance dose or BioTe ADK 10 for 3 months then transition to BioTe ADK 5 daily for maintenance. All adults should be on Vitamin D/K2 unless vitamin d levels are above 100. In PREGNANCY you take vitamin D3 alone (not extra vitamin A or K).

  • Vitamin B12 and methylation is essential for: producing energy at a cellular level, helping to prevent cancer, processing chemicals and toxins, building neurotransmitters (dopamine, serotonin, epinephrine), producing a protective coating on our nerves to prevent neuropathy in diabetes, processing and metabolizing hormones, and building immune cells, DNA, and RNA. Goal is for vitamin B12 level to be > 650 and for the patient to have no symptoms of poor methylation. About 60% of North Americans do not convert inactive folate to active 5-MTHF which is known as MTHFR. MTHFR (methylenetetrahydrofolate reductase) is an enzyme that breaks down amino acids homocysteine and folate. Homocysteine is an amino acid that works to maintain the body’s cells. Folate is a B vitamin needed to make red and white blood cells in the bone marrow. Folate also converts carbohydrates into energy and produces DNA and RNA so both play an important role in the body’s daily functions. MTHFR gene mutations can lead to high levels of homocysteine in the blood and low levels of folate and other vitamins, mostly B vitamins. An increase in homocysteine levels caused by MTHFR can lead to hypertension, blood clots, strokes, heart attacks, pregnancy loss, nerve pain, migraines, depression, anxiety, bipolar disorder, schizophrenia, and leukemia. Some symptoms of MTHFR gene mutations include: ADHD, autism, autoimmune thyroid disease, cardiovascular disease, chronic fatigue, IBS, PCOS. Treatment of MTHFR deficiency involves taking B complex vitamins along with a methyltetrahydrofolate supplement. Mighty Methylfolate has specific B vitamins for the homocysteine cycle, folate instead of folic acid, and is the same a pharmaceutical deplin! Typical Dosing: 1 capsule of Mighty Methylfolate per day with food along with OrthoMAX and Dynamic D-K2 to optimize vitamin d levels as well. Another option is Methyl Factors Plus from BioTe. If elevated homocysteine level or B12 level < 300 take 3 tabs per day with food, B12 level < 500 take 2 tabs of BioTe Methyl Factors Plus per day with food, and if symptoms and B12 level > 500 take 1 tab of BioTe Methyl Factors Plus per day with food.

  • Iron Deficiency is essential to physiologic function of the body as it has a role in red blood cell production. Deficiency can cause fatigue, decreased libido, migraines, loss of balance, hair loss, nail changes, and heart failure. The greatest source of women’s iron loss is menstrual bleeding, but other sources like GI bleeding are common in men and women. Goal ferritin levels are at least 50, but levels closer to 100 are preferred. Ferritin levels of less than 20 can be dangerous and ultimately lead to heart failure. Patients with ferritin levels this low should consider an iron infusion as it is more difficult to absorb iron from your gut at levels this low. Typical Dosing: Elemental iron should be taken at night before bed on an empty stomach or with crackers or orange juice as vitamin c aides in absorption of iron. Do not take with any dairy, vitamin d3, or magnesium.
